I have successfully uploaded my favicon to my root directory, and when I go to http://www.oa-ne.99k.org/favicon.ico, it appears. How do I get this on all pages?

My Firefox displays the Favicon (see screenshot below):

Your Firefox probably cached/memorized the page when there was no favicon in place yet which may be the reason why it doesn't display. Emptying cache, reloading or resetting Firefox or something similar should have it displaying.

BTW I think your favicon is a bit too big in size (it is 615x568px, they are usually 32x32px) and filesize (60KB, usually 4KB)
